To schedule your first visit, please fill out the registration form.  The registration form will lead to a scheduling link where you can specify a preferred appointment time. Dr. Gest will follow up and confirm your appointment. Please make sure to check out the Notice of Privacy Practices below as well as the Notice of Right to a Good Faith Estimate.

Cancellation Policy

Because each appointment time slot provides valuable physical therapy to community members, cancellations must be made 24 hours in advance so that that time slot can be made available to others. There is a $25 no-show fee for all cancellations made less than 24 hours in advance except for individuals with Medicaid insurance. Cancellations for reasons such as illness, weather, or other unusual circumstances will not be charged the no-show fee. 

​Repeat No-Show Policy

Clients with multiple no-shows without prior notice may be subject to being discharged from physical therapy either temporarily or permanently.
Foundations Physical Therapy has two locations, one located in downtown Flagstaff, and the other at the Flagstaff Birth Collective.
